The Change of Details Request is used to submit a single request to one or more Product Providers to apply one or more Change of Details to their current Agency records for either an individual or firm.
Individual Request Item Overview
This section describes the data that needs to be entered for each Request Item in a Change of Details Request, from the ‘Select Change Details’ box you can change details of an individual.
Individual Change Sections:
• Individual Name Details
• Individual Address Details
• Other Individual Details
• Reason for Change
The ‘Change Type’ box allows you to select one of the following three options:
• Change – Amendments to any details that you wish the Product Provider to update
• New Details – Additional information you wish to have added e.g. an additional address
• Retired Details – Information you wish to have removed by the Product Provider
Once you have selected the appropriate options click on ‘Select’ to proceed.
Changes to an Individuals’ Details Section
After clicking on ‘select’ to proceed you will now see the page below. Each of the shaded bars may be clicked on to expand or collapse it as necessary; the ‘Individual Name Details’ section will be expanded to start with.
In each section the fields in the left column are to be completed with the updated information you wish to send to the provider and the fields in the right column under ‘Individual identifier’ must be completed with the details already held by the Product Providers.
One of the following three options may be selected for the address type change:
• Correspondence
• Remuneration Statement
• Correspondence & Remuneration Statement
Other Individual Details
This section enables you to change Telephone, Mobile, Email Address and Web Address.
Reason for Change
This section captures any additional information that may be of use to Product Providers in
processing this Request Item
Once you have entered all the updated information, click ‘OK’ and proceed to the ‘Request Summary Page’. This page allows you to review the information you have completed on the previous pages.
If you have finished adding individual requests and have no firm requests to submit, click the ‘+ Add’ button in ‘Selected Product Providers’ section and go to the next screen to complete the submission of your request.
